Amaka Consulting provides technical assistance to community organizations during new service definition, staff development, QA/QI planning, and compliance and workflow management. Project Title: INTEGRATING SOLUTION-BASED CASEWORK (SBC) INTO GRAHAM WINDHAM INC
1. ISSUE
Graham Windham Inc uses several strategies to provide client-centered services. The purpose of this engagement was to create a singular approach that encompasses all of the agency's support philosophies.

Integrating SBC into foster care for kids and bridge to health programs
3. RESULT
As Graham Windham's Program Performance and Planning Consultant, Amaka guided Graham Windham Inc into the development of the "One Graham Care Model."
The model brought together all of the agency's primary services under one operational programming goal using SBC and an integrated case management paradigm. The One Graham Care Model aligned services portfolios with the agency's overall objectives and improved performance by 60 percent.
While SBC prioritized working in partnership with families focusing on generating pragmatic solutions based largely on families' input and involvement, the integrated care management (ICM) approach ensured that every facet of the family well-being including external parties and resources relevant to the solution are brought in sync and coordinated with the family at the front and center.
